Actually, this is not as uncommon as you would think.
I have had many patients who have been afflicted with gouty attacks come into my office for treatment. I'll examine their area of complaint -- sure, it looks like gout to me. Then, I'll check their uric acid level in the blood. Sometimes, it may come back elevated and confirm the gout, and yet sometimes it has come back in a "normal" range. I began to understand over time that at a normal level, the patient can still have a gout attack because their threshold for an attack may lay within the "normal" level. So, needless to say, it just is what it is. The other test (more uncomfortable) is to aspirate the fluid within the joint to look for uric acid crystals. (I don't usually do this though). I'll typically give the patient a steroid pack or NSAIDs and get them comfortable enough.
