Gout is a form of arthritis that affects the joints and usually (more than 90%) the joint that gets the very first gouty inflammation is the big toe on either side of the feet.
Gout is the direct result of excessive accumulation of uric acid in the body and it is usually diagnosed through the elevated uric acid level in the blood test. Actually it is not the uric acid in the blood stream that causes the gout attack, is it the uric acid OUTSIDE the blood stream that triggers the gouty inflammation.
Uric acid is the by-products of the breaking down of purine in the liver. Under healthy body condition, these unwanted uric acid will be carried by the blood to the kidneys for disposal through urine. Problem arises when something went wrong along this purine uric acid disposal cycle.
Here Are 3 Possible causes:
1. Too much consumption of high purine foods which are usually high protein diet such as red meat, organs, seafood etc. All these excess consumption of protein (which will be transformed to urea for disposal) and purine will greatly burden the workload of the liver and kidneys. When the input is more than the output capacity, this will cause the elevation of uric acid levels in the body.
2. Some other health complications such as diabetes, hypertension, kidney stones, and hepatitis, that affects the functionality of the organs involved in the uric acid disposal route.
3. Dehydration. Once common habit among most gout patients is that they don't drink enough water in their daily life. All they drink are 'synthetic drinks' such as soft drinks, coffee, tea, beer and liquor. All of them are dehydrating agents! They 'suck' water out from the cells and interstitial fluid which increases the concentration of uric acid level in the body.
When the uric acid levels in the the body fluids increase, it will force the uric acid to seep deeper and further away from the blood stream until it reaches the synovial fluid in the joints. And when the concentration of uric acid in the synovial fluid reaches the saturation point where no more uric acid can be dissolved, crystallization happens and tiny needle like urate crystal will form in within the joints.
For the immune system, these urate crystals will be identified as foreign substances that invade the body and the body will call for more white blood cells enter the affected joint to attack the urate crystals. And it is this immune reaction that causes the pains and swellings as seen in a gout attack.
