Gout is extremely painful. But it is easily diagnosed with a blood test. If you suspect it is gout eat cherries and drink a lot of water. No Tylenol just ibuprofin ( advil)

Yes it is very painful... The tissue is probably all inflamed... movement is probably agony.
It's beyond ibuprofen. Ibuprofen isn't even going to help him get through the day without pain even if he stays in bed. I can't imagine working with gout or an infected cut... especially if he'd be working with that arm and using that joint.
I have a high tolerance for dealing with pain, I've worked with a bad back after a car accident and I broke my foot once when pregnant and refused to take pain meds, and I still walked to work and my doctor was just astonished I was holding up... so I kid you not, I can take more than a few aches and get through the day.
But I had an infected toe once, just from the nail which got ripped back, and I couldn't even put the foot down without my eyes tearing.
I have a good friend, too, who has gout, it's agony whenever he gets an inflammation. Another person who is no stranger to pain, never let an ache or injury stop him from walking to work even if he had to go on a cane (which he did for almost a year after a pulled ligament). Always in good spirits, most people who didn't know him would never know if he was hurting as he doesn't complain. But when gout inflammations come, he is down and out for the duration.
I don't know how good your husband's tolerance of pain is (I know mine is usually a baby about it, lol), but if it's gout or infection, he's probably hurting pretty badly.
