My mother suffers from gout, too. Since the joint is alreay read and inflamed I would think something cool might feel better than heat. Ibuprofen helps, and there are also prescription gout medications both to help acute attacks and prevent more in the future. The best thing that has helped my mom when she has a bad attack has been cortisone shots directly into the affected joint, but not many doctors are skilled at that or willing to do it instead of the oral medication, which my mother is allergic to.

Gout is usually in a lower joint such as a big toe, ankle, or knee. But the pain your mom feels sure could be gout.

Heat is best. Cold will make things contract, and that would be bad. The pain comes from the immune system attacking the small crystal of uric acid, and then inflammation sets in, and then the horrible pain starts.

Applying heat is not enough to help your mom with gout. She needs to also watch what she eats, and help her body get rid of uric acid. One easy thing to do is have her drink every day, 1 oz of water for every 2 lbs of body weight. Yep, that's alot of water each day, but it helps the kidneys to get rid of the uric acid.

There's of course a LOT more that can be done with diet, prescription meds, and natural herbal remedies.
