Pills should help. Maybe ask Nitram, I don't know if its's easy to contact through mail via Yahoo Answers, he sometimes appears on the 'Alternative medicine' section, and knows a lot about quackery and science, so I am sure he will know what kind of doctor and pills will help for sure.

You can try going to the ER but that is a crap shoot. You might get a doctor that will help and you might not. It's worth a shot though.
First thing is to get your body in order. No matter what, you need to get the diet and exercise going. That will help eventually even though it may be painful to start. Also, cut out the soda. I'm not sure what you are drinking but it's a safe bet that it's probably a cola. Even diet soda needs to be kept to a reasonable amount.
You also need sleep. See if your doctor will prescribe something to help you out. There are a number of medications out there, so if one doesn't work have them try another.
If all the tests come back negative and you're still in pain, go see a pain management doctor.

I've done battle with gout for 14+ years and I have it under control. Here's what I know about gout that will hopefully apply to you and help you.
Bottom line, the human body needs to be able to get rid of uric acid. If it doesn't get rid of enough of it, then the uric acid pools in the joints, and crystals form, and the horrible pain begins. Usually, gout occurs in lower extremities, such as big toe, ankle, knee. But it sure can occur in elbow and probably in other joints too. It's very possible that you have more than one malady going in your body.

So, how do you help the body to get rid of uric acid?
1) Drink lots of water, 1oz for every 2 lbs of body weight daily
2) Get a script for allopurinol, very inexpensive and no side effects
3) Stop drinking things that dehydrate the body, or things that are acidic. So reduce big time or cut out completely: beer, coffee, soda, and high fructose corn syrup
4) One thing that helped me get more fruits/veggies into my diet, is to drink V8 VFusion. It's tasty and comes in many flavors
5) Eat lots of sour dairy, which is sour cream, cottage cheese, plain yogurt. The latter, you can make smoothies with orange juice, pineapple, bananas, and frozen berries. Good stuff.
6) Take celery seed pills. Helps the body to not create so much uric acid.
Educate yourself on gout. Don't depend on the doctor, they need to know a little bit about a TON of subjects, so chances are the doc won't know much about gout.
Anything anti-inflammatory, whether it's food or drink or pills or whatever, ya need to do it. This includes cherries, pineapple, lots of water, ibuprofen.
