Nothing precipitated the swelling this time, just a feeling of "not being right" and exercise.
The same thing happened a year ago (pain and swelling for a few days), then it was fine for the year.
Could this be gout in the ankle that was injured and weak ... I rarely drink alcohol.

yes, soft-tissue injury can be permanent. The soft-tissue is damaged and repaired with scar tissue which limits the function of the joint. Loss of joint function over a period of time, leads to destruction of that joint, eventually arthritis.
start a stretching routine to help alleviate fibrotic ligament contracture,, to hopefully restore lost joint function.

Yes. I severly sprained my ankle 3 years ago too. I was on crutches and had to have physical therapy for 3 months. It was iced and proped up, no exercise except for the therapy. I couldn't put much weight on at the time. To this day it still doesn't feel right when I exercise or walk alot. It's just something that is going to flair up every now and then. I've learnt not to expect too much out of this ankle. Hope this helps!
